Pemphigus and Pemphigoid
Course Description

Comparative Overview
This section provides a detailed comparison of Pemphigus Vulgaris (PV) and Mucous Membrane Pemphigoid (MMP), focusing on their distinct autoimmune mechanisms and clinical presentations. PV affects desmosomes, leading to fragile blisters, while MMP targets the basement membrane, resulting in more resilient blisters.

PV: Diagnosis
Pemphigus Vulgaris is diagnosed through clinical examination, biopsy, and immunofluorescence studies. This section covers key diagnostic criteria, including the presence of mucosal erosions and intercellular IgG/C3 deposition, which are very important for differentiating PV from other blistering diseases.

PV: Pathophysiology
PV is characterised by the production of autoantibodies against desmoglein proteins, leading to loss of cell adhesion (acantholysis) and blister formation. This section looks into the genetic factors, immune responses, and environmental triggers that contribute to PV development.

PV: Management
Management of PV typically involves systemic corticosteroids, steroid-sparing agents, and biologic therapies like rituximab. This section looks at treatment strategies, including topical therapies for oral lesions and the need for ongoing monitoring of side effects and disease activity.

MMP: Diagnosis
MMP is diagnosed based on the presence of mucosal erosions, desquamative gingivitis, and linear IgG/C3 deposition along the basement membrane. This section highlights the importance of biopsy and immunofluorescence for confirming the diagnosis.

MMP: Pathophysiology
MMP involves the autoimmune targeting of basement membrane proteins, leading to subepithelial blister formation. This section discusses the immune response, focusing on the role of BP180 and BP230 in maintaining tissue integrity and how their disruption leads to blistering.

MMP: Management
Management of MMP includes topical and systemic corticosteroids, immunosuppressive agents, and careful monitoring for complications such as scarring and vision loss. Dental professionals play a key role in managing oral lesions and collaborating with specialists for comprehensive care.

Course Completion
Participants will take a multiple-choice exam and provide feedback. Upon passing, they will receive a GDC-compliant CPD certificate. The course emphasises a multidisciplinary approach to diagnosing and managing Pemphigus Vulgaris and Mucous Membrane Pemphigoid in dental practice.
